___ Revenue (income or sales from the business) Please bring bank statements for all 12 months.
___ Advertising Please bring copies of checks or check stubs for all expenses.
___ Mileage (you need to keep a log to include mileage to the bank to make deposits)

___ Contract Labor (I need the name, social, address, and total amount paid for the year to each person)
___ Equipment purchased (date of purchase and amount paid is needed)
___ Liability insurance (not auto insurance)
___ Legal and professional services (lawyers, accountants, notaries, etc.)
___ Office expense (office supplies, postage, etc.)
___ Rent of vehicles, machinery and equipment
___ Repairs and maintenance (that are not vehicle repairs and maintenance)
___ Rent on office
___ Materials, supplies, small tools
___ Taxes and licenses
___ Travel (hotel, car rental, airfare, bus fare, train fare—do not include meals)

___Bank Charges on business account
___Mobile phone if used for business

___ Uniforms Must not be clothes that you can wear as regular clothes. Suits are not uniforms.
___ Office in home expenses. Need square footage of the part of the home you use exclusively for business, the square footage used part time and hours used, total square footage of home. Need rent, mortgage interest and property taxes, homeowners or renters insurance, repairs and maintenance to home, utilities (CPS, water, garbage), HOA or condo fees.
