1099 CHECKLIST
Please put your contact information on a cover sheet. In order to prepare the 1099s, I will need the following information:

____ Federal tax identification number
____ Address
____ For each contractor or non-employee, I will need their name, social security number, address, and total amount that you paid them for all of 2011.
